Completing your PRS enrolment forms
Below are samples of completed PRS Enrolment forms for easy reference as you’re completing your forms – at work, at home or on-the-go. For additional information on selecting investments, click here.
- PRS Joint Account Opening Form
- PRS Contribution Form
- Investor Suitability Assessment Form (Only required for Do-it-myself)
Once you’re done completing your enrolment forms, please submit them to your HR team along with a photocopy of your NRIC (with the front and reverse side printed on the same page). They provide them to us for processing. Typically, within a month, you’ll notice your contributions are being deducted from your payroll.

Final tips
Do check that all mandatory fields have been completed for each form
Ensure all forms are signed at the right places as per the sample form provided
Attached a photocopy of your NRIC (with the front and reserve side printed on the same page)
Submit completed forms to your HR and they will call us for processing.
